We will do our best to accomodate same-day appointments for all patients! If this is your first visit to our clinic, please complete the paper work located on the Forms to Download page. You will need (2) documents completed: the new patient registration form and a health history form (either adult or pediatric). If you have medical records at another doctors office or hospital that you would like us to have, you can complete the authorization to release medical records form as well. We hope to have all these forms available for online completion before the end of 2008. |
